KEY EXAMPLES
CASE MANAGEMENT:
(1) Case Management offered for 3 years post
release – The Way Home, Sussex.
(4) Walk through every step, including legal/court
system, medical help – House of Pride, Kent.
(14) Development and application of Individualized
Service Plan focuses on all of the above needs. Service begins 3 months
prior to release and continues 9 months following – DCJ Adult Reentry
Mentoring Program, NCC.
(17)Seamless transition for HIV-positive offenders
to community, 30 days prior to and after release–DCJ HIV/AIDS Program,
Statewide.
SPIRITUAL COMPONENT:
(8) Establish spiritual support by using selected and committed people
as mentors, hosts, etc. - Churches Take a Corner, NCC.
COMMUNITY INVOLVEMENT:
(9) Mentoring and teaching men and women in
recovery homes and on probation – Churches Take a Corner, NCC.
(15) Coordinate support by volunteer mentors from
community - DCJ Adult Reentry Mentoring Program, NCC (See
Drug/Alcohol…).
(5) Clients earn their keep by providing service
(lawn care, mentoring, painting, etc.) to others in the community –
House of Pride, Kent.
HOUSING:
(2) Help with immediate shelter, food, and clothing
needs. Soon adding transitional housing. - The Way Home, Sussex.
(6) Housing provisions for individuals faced with
hardships or the inability to be self-sufficient - House of Pride, Kent.
(10) Seven different sites of transitional housing,
each offering stay of up to 9 months and helpful programs – Friendship
House, NCC.
DRUG/ALCOHOL EDUCATION AND RECOVERY:
(16) Pairs each participating offender with one
mentor from the community and one mentor who has successfully completed
the Key West Program and continues to lead a substance-free and
crime-free life. – DCJ Adult Reentry Mentoring Program, NCC.
(20) On-site substance abuse education program for
60-90 days – Sojourners’ Place, Statewide.
TRANSPORTATION
(3) Transportation to probation and other
appointments, initial job transportation, and one-on-one counseling -
The Way Home, Sussex.
(13) Bus tickets available to clients for
agency-arranged appointments and activities- Samaritan Outreach, NCC.
PRE-EMPLOYMENT
(11) Acquire Identification, including the payment
of the required fee – Friendship House, NCC.
(21 & 22) GED, Life skills, and Vocational
Rehabilitation: Testing, Training, and Job Readiness - Sojourners’
Place, Statewide.
EMPLOYMENT
(7) Clients have paid off more than $100,000 in
fines to State, etc., over the past three years - House of Pride, Kent
(See Community…).
(12) Full employment through first-time job program
– Friendship House, NCC.
(18 & 19) Job Search Development and Job Placement
– DCJ Prison To Work, Statewide.

Includes elements of both Integrated and
Linked types through one diverse program or multiple, distinct programs.
Pre-requisite:
Suggests that the agency recognizes the importance and
priority of a service area in which they do not offer programming, so
the agency requires clients to address and satisfy this need before
admission into their program.
No Service:
Suggests that the agency either did not have
information available the service or does not provide it, but would
likely encourage and aid the client, resources permitting, to engage
other agencies that specialize in such service.
